The scheme provides a follow-up telephone and home visiting service to support people over 65 returning to their homes after being discharged from the Accident and Emergency Department of North Manchester General Hospital. Like St Joseph’s Welfare Centre, St Joseph’s A&E Befriending Scheme is funded by the local council and seeks to ascertain that the patient is coping following their stay and signposting them to relevant services where appropriate.
